Starting a Case
Application (General)
Starts most family court cases — used to ask the court for orders about parenting, support, property, or divorce combined with other claims.
Answer
The responding party’s reply to an Application — sets out their position and any claims of their own.
Divorce
Application (Divorce)
Used to start a divorce-only case (a “simple” or “joint” divorce) where no other claims are being made.
Divorce Order
The order that grants the divorce.
Affidavit for Divorce
Sworn evidence supporting an uncontested divorce, used to ask the court to grant it in writing.
Financial Disclosure
Financial Statement (Support Claims)
Required financial disclosure where only support is claimed — income, expenses, assets, and debts.
Financial Statement (Property and Support Claims)
Required financial disclosure where property division (equalization) and/or support is claimed.
Certificate of Financial Disclosure
Lists the financial documents each party has provided.
Motions & Orders
Notice of Motion
Used to ask the court for an order before the case is finished (a motion).
Affidavit (General)
Sworn evidence filed in support of a motion or other step in the case.
Motion Form
Used for procedural or uncomplicated motions, often in writing without attending court.
Order (General)
The formal document recording the court’s order.
Parenting & Support
Affidavit (Decision-Making Responsibility, Parenting Time, Contact)
Required affidavit when seeking parenting orders — sets out the parenting history and plan.
Service & Filing
Affidavit of Service
Proves to the court that documents were properly served on the other party.
Acknowledgment of Service
Signed by the party served to confirm they received the documents.
